Elbow joint is made up of three bones i.e. the humerus bone of the upper arm and the radius and ulna at the forearm. A hinge is formed by the ulna and humerus which facilitates straightening and bending of the arm.
When the elbow joint sustains a major injury or is impaired completely resulting out of degenerative disease and no technique is able to provide relief then replacement surgery is the last option that is undertaken by the elbow surgeon to repair it with metal and plastic implants.
In cases where replacement surgery fails, the elbow surgeon has to replace the damaged artificial elbow again and this procedure is termed as revision elbow replacement surgery.
After the decision of the surgery has been reached upon, the elbow surgeon has an in depth discussion with the patient regarding potential risks and benefits of the surgery. All anxieties and questions of the patient are tended to and to determine that the patient is healthy to undergo the surgery, a few tests are run on the patient by his regular doctor. Once all pre operative formalities are completed the surgery is gone ahead with.
During the revision surgery, after anesthesia, the arm is prepared and incisions are made depending on the location of the previous scars. The nerves and the tendons in the joint are exposed and the artificial implant is removed delicately along with the remains of previous bone cement. Then components of the three bones are fitted in one by one and cemented in. After each bone has been replaced by prosthesis, the elbow surgeon sews back the incisions and places the elbow in a heavy bandage and splint.
After the surgery, the elbow is splinted either straight or slightly bent and the patient is kept under close monitoring under continuous medications to reduce swelling and post surgical pain. A follow up visit is scheduled within the first week as the surgeon wants to assess the progress of the elbow in terms of healing and mobility.
Complications are a probability with all surgical procedures. The complications that may occur during or after revision surgery may be damage to the nerve or blood vessel in the elbow, infection, metal implant failure or allergy or loosening of the implant.
Under instructions and guidance of an efficient physical or occupational therapist a comprehensive rehabilitation program is formulated to help restore stability, strength and mobility in the elbow. It may comprise of one or all of the following- gentle in motion exercises, exercises on highly technological rehabilitating machines, some changes in the surroundings and learning how to adapt the elbow to them is all learnt by the patient under the therapist.
This is not a normal course of treatment that is taken up by the elbow surgeon. It is only adopted when a previous replacement surgery has failed or the artificial implant has become soiled due to wear or tear. Thus on a general basis the benefits of revision surgery lie in the accessibility to rejuvenate the elbow joint by a new implant easily.
